At Freeform Physical Therapy, wellness is the top priority in all treatment plans. Wellness is an active and ongoing process that can adapt to changing physical, emotional, and cognitive demands. Our emphasis on wellness allows us to meet patients where they are and create dynamic treatment plans that evolve as patients progress. This ongoing and collaborative effort focuses on more than healing injuries - it lays the foundation for longevity of performance, self-management of impairments, and participation in activities of our patients’ choice.

Performance takes on many definitions in this practice. Whether referring to an athlete performing during competition or performing daily activities without limitation, the goal is to help patients reach the top of their game.

The office is in Newark, OH and telehealth appointments are available for patients in Ohio, California, Georgia, Utah, and Kansas. All physical therapy appointments are one-on-one with Dr. Claire Kopko. Patients can expect excellent continuity of care and a highly accessible provider between sessions.

Freeform Physical Therapy is out of network with all insurance providers. This allows Dr. Kopko to create plans of care guided by patient goals and clinical prognosis rather than by a pre-allocated number of visits and a corporation’s determination of medical necessity. This also allows patients to access physical therapy treatment without referral from a physician.

Freeform Physical Therapy’s emphasis on wellness, performance, and patient self-advocacy has created a more “free form” model of physical therapy treatment that allows patients to drive their own care based on personal goals, scheduling preferences, and current level of activity.

The Freeform model truly allows Dr. Kopko to meet patients where they are and consider where they want to go with their physical therapy.

MEET YOUR PROVIDER

Claire Kopko

Claire Kopko is a doctor of physical therapy with licenses in Ohio, California, Utah, Georgia, Indiana, Pennsylvania, and Kansas. Her clinical interests are vast, including orthopedics, manual therapy, sports medicine, pediatrics, and health and wellness. Dr. Kopko’s passion for an active lifestyle is what drove her to the field of physical therapy. She is eager to share her knowledge and instill the same passion for health and activity in her patients.

Dr. Kopko obtained her Bachelor of Science in Biology with a concentration in neuroscience from Denison University in 2008. During her undergraduate career, she was a four-year starter for the Big Red softball team and a National Strength & Conditioning Coaches’ Association All-American athlete. She spent two years as an assistant coach and creating a strength and conditioning program for the softball team at Division III Washington & Jefferson College.

Dr. Kopko earned her Doctorate of Physical Therapy (DPT) from Chatham University in 2012. She completed her orthopedic clinical residency through Movement for Life and the University of Southern California, culminating in obtaining the designation of Orthopedic Clinical Specialist (OCS) in 2015.

Dr. Kopko has clinical experience in a variety of settings, as she began her career in a private outpatient clinic with a large population of geriatric patients and currently works with special needs children in both school and teletherapy settings. She spent four years as the physical therapist in the Center for Limb Lengthening and Reconstruction at a large children’s hospital in Columbus, OH. During that time, she devised rehabilitation protocols for children and adults undergoing correction of limb deformities and provided education to her colleagues as to how to treat this specific population.

Dr. Kopko's preferred fitness activities are hiking and strength training. She is currently trying her hand at tennis and loves any water activity. Her husband, Nick Showman, is the owner of Showtime Strength and Performance. They live in Granville, OH with their dog and enjoy traveling, listening to live music, and reading.
